Is this an unintended consequence of the insurance companies' (and Medicare's) severe cutbacks in what they will pay for drugs? In my case, the insurance tried to increase my copay for a drug from $10 to $35 per month, even though the alternate drug they wanted me to use had, in my case, life-threatening side effects. When I got nowhere on an appeal I sued them, and they caved. But people less assertive than I might just have done without the drug, or gone without other essentials to pay for it.

We're getting closer and closer to a very dangerous position where bookkeepers determine the quality and availability of medical care and therapies.
