The definition of communism has been changed by 20th century history, in the wake of Stalin, Mao, and the many other murderous regimes. Any large-scale exercises in socialism, including those in Russia and China, were quickly abandon under a torrent of famine and counter-revolution. But communism today, generally means the ultra-corrupt systems like the former Soviet Union, where universal health care was taken for granted, as was the ineffectiveness of the health care system.
