Thanks for the great post. This answers
alot of questions. I have one more, though:

Given the RBOC pace (or lack thereof) and the
presence of many local internet service providers--
Is there any possibility that these ISP's could build
an urban switching infrasctructure that would supplant
the RBOC's?

In the Bay Area, with am 18000 ft reach, you would
probably need about 200-300 switching sites. Capital
investment of ?
