Had same problem with my five year-old. Got tired of waking up in middle of night to make sure he was on his back. He eventually slept on his back continuously but developed a slight flattening of the back of his head which we considered unsightly.

Had a special helmet made which straightened him out. The baby pictures are hilarious.

Expect to be asked about the helmet a few years from now. Don't yet know what to say, as I expect that making babies sleep on their backs to avoid SIDS will soon be proven a farcical old wive's tale on par with extended bed rest for heart attacks.
