First of all, let me say that I agree that it was *nearly* preventable. I think you will see design changes for new jetliners and retrofitting for older ones in very short order.

However, our system has always been, in all things, to wait for a system to fail before changing it. This applies to our highway system, automobiles, electric grid, well, everything. It makes no sense to think that all things are preventable given enough design-think and money. No, that's "Titanic thinking". Hell, do you think the bubble was preventable? I don't? Each time we get one and its inevitable aftermath, we make changes to avoid the next one, but we do not avoid the next one because we never, ever identify the true cause.
