So, in our world the idea of a country is based more on an ideal, rather than geographical boundaries.

If we're to be at war, it's certainly more convenient to be at war with a country that has a legitimate government. In that case, you can assume that all countrymen are the enemy. It may not be precisely the case, but the fuzziness can reasonably be ignored. The practical implication is that you can bomb the country to smithereens without undue concern. When the enemy is a faction or an outlaw band within a country or when the government is not legitimate, things get complicated and innocents become more of an issue.
