The ISPs have banks of, e.g., V.34 modems tied to copper and connect to users on a point to point who also have V.34; their gateways then connect to the infrastructure through, usually, T1 or T3 lines depending on the bandwidth needs. In the case of ADSL the picture changes. Users with ADSL modems connect to the switches and then the RBOC provides a T1, T3, or fiber to back to the ISP.
