For a software service solution, look at gainskeeper.com . They keep track of wash sales, splits and spin-offs, and adjust your share counts and basis automatically. Some brokers are set up to download to them directly, others you have to download to Excel and massage the data a bit.
They have a nice Schedule D report and good YTD reports.
