Depends on who you are starting the operation for, yourself and a few guys or if you are trying to make a few hundred million.

If I were a bright guy, I would look into:

1) handling riser management for buildings, decent MRC
2) competing with the UIX database by developing a Lattis (pricing) tool type system that shows who is who in telecom in a given building, also who is interconnect with what carriers.
3) Negotating with specific buildings the exclusive rights (aside from Ameritech) the ability to terminate service into those buildings (don't know if that one is legal or not)

Those are the first ideas that come to mind.
