At the same sight are one or more e-mail addresses for asking questions, making complaints, etc. No guarantees about how soon they will get back to you, though. Calling might be the best way, but remember this is THE government and all the red tape that that has come to represent. Me cynical? NAH-H-H! :)

ps. No, I don't know anything about Form "D", but keep watching. There is more than likely someone who follows this thread and knows more about Form "D" than most stock brokers who will reply
