The guys that own that last mile like T's cable or the Baby Bells...once it's set up, $5o more than covers it, IMO. One reason there's not more competition is that the Bells don't want to hurt there higher priced business broadband ISDN, T-1 and such for which they get $$$'s. Cable offers twice the speed as DSL and prices just under them...enough to get most of the business. If Bell DSL wasn't afraid or ruining that other biz, prices would be a lot more competitive.
