I don't know either about many of the points you raise -- I think we can safely rule out exercise and clean water. The research done recently seems to point to diet in genral and the type of fats we eat in particular -- we are talking about ten times the rate of prostate cancer and breast cancer. It also seems that cancer rates in Asia are going up as their diet comes to reflect some aspects of our fat intake -- this all seems to be taken increasingly seriously in the scientific community.
