The drug itself pretty clearly works for some people where nothing else does. It's not a miracle drug and it will likely take quite a few years to learn to use it properly - to really figure out what combination works best.

I do not believe it is a scam. Signals from the FDA can often be misread - IMCL is by no means the first company to have a refusal to file. This is nothing at all like Enron. My one concern here is that normally companies like to deal with the FDA in private, whereas now everything is going to be out in the daylight. The FDA might well become more defensive and harden its position,

A little more on combination therapy in cancer. Most regimens now have multiple drugs in them. Figuring out the right combinations and dosages is very hard, but can make an unbelievable difference. A good example is testicular cancer. This went from near death sentence to near definite cure in the course of about a decade, with no new drugs. All that happened was that people eventually figured out the correct combinations and dosages. The lesson is that it's hard to know the full impact of any cancer drug when it's brand new.
