According to an article in the Washington Post this a follow on the policy set forth in 1996 during Clinton's Presidency that outlined potential threats that could require nuclear targeting as a result of the demise of the cold war. Seems that the only thing really changed is adding Syria and Iran. I am not sure whether using tactical nukes was considered in the previous strategy vs the current. If they were not then this is a significant and dangerous change.
