"Restoring the moral balance of the universe, at a minimum, means showing respect for the principles that order our lives, deploring an egregious violation of them, and taking affirmative steps to not let someone shirk responsibility for his actions........

Yes.

"I would say that the penance is purgative, somewhat like giving an emetic or excising a tumor."

The excising of the tumor is likely to cause a painful hardship on the patient. I doubt that the patient would view the doctor's behavior as retributive.

Lets imagine that the patients life style was such that it contibuted to the likelyhood of tumeric growths. The experience may, in fact, be a punishing experience for the patient. Things are experienced in different ways by different people. Someone else might view the same experience as a relief of trama and a chance to start fresh. An outsider who knew about the excesses of the patient may be thinking that the patient deserves to suffer through the hardship.

The point is, the only thing we can do as a society is send in the surgeons. Problem resolved in the most constructive way. People will make of it what they will. There is no need for us to make vengence a graduate course in medical training.
