There was an interesting History Channel documentary on the USS Liberty a while back. Since the Liberty was a minimally armed intelligence gathering ship not very close to any kind of fighting and since the US flag was clearly displayed, it is unlikely that the attack by Israeli airforces could have been anything but deliberate. US president Lyndon Johnson did have the opportunity to send planes from a carrier to defend the ship, but apparently he didn't wish the US to become involved in what was allegedly understood to be an escalating situation involving Egypt and so balked on the idea. A sad deal all the way around.
