Have to chime on the handicapped side - you can't see hip replacements, knee replacements, neurological problems, formerly broken limbs and many other elderly afflictions that make walking anywhere very difficult sometimes.

Yes, both my parents have handicapped permits and don't look visibly impaired at first glance. Trust me, most folks who have those need them.

I can't speak for the grossly obese - fortunately that's not on their list of ailments, just the usual American proportions.

As to a market solution to our car-crazy lifestyle, it's hard to undo the urban areas we have already created. For new ones, make parking hard and public transport or self-transport (bikes and walking) very convenient and people will follow the lead. I wouldn't take my car into downtown Amsterdam for anything but I don't have to - society has provided lots of choices.
