"I'm having a fetus." They know it's a baby from the beginning.

That's too cliche.

"I'm having a baby" means "I'm going to have or to give birth to a baby," which she will, indeed, do if things turn out OK for her and the incipient baby. She's not actually "having" the baby until her water breaks and contractions start. "Having" a baby means delivering it. One could say that the process of having a baby starts when one conceives it, but the actual "halving" occurs in the delivery room and what comes out is, in fact, a baby.

If you want to go with common parlance, a woman who wants a baby calls it a baby. A woman who doesn't want a baby commonly says she "is pregnant." The usage of the word, baby, is for those who are anticipating a happy event, the delivery of their baby.
