Did the person laugh out loud when the caller said this? I carry my own health insurance for myself and my husband. It costs $4000/year

It's not unrealistic to pay $20,000 a year if anyone in your family has had a health problem. I pay $18,000 a year (family of three) for a group policy with BCBS (I am a "group" of one as an independent consultant - although a good part of my income comes through Schedule D).

I'm thinking of self insuring when my capital base is large enough but I'm a few years away from that.
