I'm just guessing mind, but I suspect that the questions you've raised are exactly the same ones that all the Pension, Institution, and Mutual Fund managers who haven't got any more answers than you or I are asking.

Considering the amount of money they lost when the last Bull was butchered, they may, and certainly should be, loath to make a move without a virtual guarantee of their principals' principle. More than likely, a substantial number of them simply can't afford to risk being "early" to the next party. ...Which naturally further delays the festivities.

Churning, channeling, program trading. Whatever you want to call it, the chronic low volumes seem to suggest that there are a good many practitioners who won't move a significant degree on either the sell or buy side unless forced to do so. Maybe "disciplined" is the better description of the situation.

Beats me.
