one interesting thing I heard this morning was that cost cutting is the usual suspect in many of these catastrophes. In the end costs cutting costs more because you have to go back and rework the section, more often than not. He said cost cutting added to the Challenger disaster because the engineers originally wanted different solid rocket boosters. No one knows what caused this crash yet.
