I think you are on the right track here. On takeoff, some of the insulation on the main fuel tank showered down on to the shuttle at tremendous speed, possibly damaging some of the "heat tiles" or the left wing of the craft, which may have played a part in the disaster.... In any case, NASA is playing with fire here, using relatively "ancient" technology and shuttle craft to place human beings into earth orbit. It's time for them to move on to better things IMO.
