In all likelihood the damage inflicted on the underside of the left wing at launch will turn out to to be the cause of this tragedy. HOWEVER those "engineers" who assessed the situation did not see fit to have either the robotic arm with its camera or better still an astronaut actually examine the wing during an EVA during the 10 days Columbia was docked at the station. I find this almost incredible considering what was at stake. If tiles were found to be missing then we would assume that the astronauts with their spacecraft would still be at the ISS today while NASA worked out a solution. There is gross negligence in this story somewhere perhaps even criminal negligence.
