Tiles problem was common knowledge for ages. It is sheer stupidity for NASA to let this happen. My experiment with NASA (a small biological experiment) was cancelled because we couldn't give 100% assurance in 1967. We need that 100% assurance brought back, along with 200% inspection.
NASA should have refuse to fly shuttles unless funding for proper assurance is obtained.
It seems like we're growing into too tolerant a society for people who shrug off responsibility.
