Yogi - Having been in similar situations I can say that it is easy to say the best effort was not made. Not so easy to make that judgement in real life without 20/20 hindsight. It isn't like the only potential life threatening problem is the tiles. To get everything to the level that you are talking about would probably double or triple the cost of every shuttle flight - if not vastly more. And this from what has to be one of the most inefficient of all government agencies. The shuttle is very very complicated and the energies being dealt with are absolutely huge.

On any problem with big enough consequences you will ALWAYS find a large number of people who 'predicted' it. But as some wags have said about some financial analysts - they have predicted 10 of the last 3 recessions. And such 'fortune tellers' are worth essentially nothing.
