Not at all sure what Korea means by "total war", but I have been mulling it over ever since I first read it. Seems to me it could mean one of maybe three or four things:

1) Most likely - Kim Jung Il seems to think that if the US gets involved in war with North Korea, that China would come in on North Korea's side, and that Japan would get swept up into it, and maybe even Russia.

2) Less likely but still plausible - like Iraq, North Korea is said to have smallpox. Releasing smallpox into the world at large again would cause untold deaths, potentially billions.

3) Psychiatric analysis - the Fearless Leader is a narcissistic psychotic with delusions of grandeur. He's attempted to infantilize the entire country of North Korea in order to dominate them completely, and doesn't understand that his ravings don't translate well into English.

4) Whatever he really meant was sane but doesn't translate well into English.
