Usually, if one washes the fish well and cooks it properly the germs will be killed. I suppose, however, if the water is contaminated with sewage or polluted effluent then one has to be a lot more careful. Maybe it's best, in such circumstances, to avoid eating anything caught in that place altogether. Of course, fish can also be contaminated with heavy metals, like tuna is with mercury, but that's another story.
