I too have heard those reports but you can't be too sure about either sides word after an event like this. Everyone is trying to cover their butts.
What reason does the management at the previous gigs where there were no problems have to cover their butts?

THis is what convinces me the band did it without notice: It's these OTHER guys who have no particular reason to favor either side saying they had done it before.

Well....I'm not as sure about that as you are.
Maybe. Some would probably do practically anything to pull in a crowd.

Neither side wants to be fingered and I suspect we'll find out that this wasn't the first show at that venue that had pyrotechnics.
If THAT turns it to be true it changes the whole situation.

Sometimes an accident is just an accident even when people die.
"Accident" is one thing. Acting recklessy without regard to possible consequences is quite another. That's a felony called manslaugher if someone is killed.

This is from the CA penal code:
192. Manslaughter is the unlawful killing of a human being without
malice. It is of three kinds:
(a) Voluntary--upon a sudden quarrel or heat of passion.
(b) Involuntary--in the commission of an unlawful act, not
amounting to felony; or in the commission of a lawful act which might
produce death, in an unlawful manner, or without due caution and
circumspection.
This subdivision shall not apply to acts committed
in the driving of a vehicle.
(c) Vehicular--
