smallest possible munition for the job is selected.

Of course you are talking about command strategic targetting strikes. In the heat of the battle and on instantaneous confrontations the 20 year old on the end of a tank cannon doesn't have that process available to him. In these cases, innocents are at extreme risk, obviously. Presumably the Palestine Hotel demonstrates that clearly.
