On the photograph in that NEJM article the skin lesions appear to be pretty bad, but the authors do not think that monkeypox is "a serious epidemic threat". I hope they are right.

No one really knows whether or not millions of people who many years ago were vaccinated against smallpox still maintain their immunity. Supposedly, vaccination against smallpox conveys immunity against monkeypox. If previously vaccinated people can get sick with monkeypox, that may indicate that they may no longer be protected against smallpox either.
