There are a number of intermediate steps you could take.

One would be to write to them and say "I am prepared to discuss with you settlement of my potential claim against you for providing a defective vehicle in violation of your implied contract to provide a vehicle in good working order." Another is to write a letter demanding that they retain the tire in its current condition until your accident investigator can inspect it and determine to what extent the rental company or the tire manufacturer may be liable for your damages. Another, if you have a friend who is a lawyer, is just to have them write a quick letter inviting them to resolve the matter quickly by making an offer to resolve your claim agains them without the need to resort to litigation.

Another is to write a report of the incident in the form of a press release with the headline "[Name of company] Rents Defective Vehicle to Elderly Woman" (if you don't object to the term elderly" and outlines the story, mark "Draft" on it prominently, and send it to the company headquarters and ask the company whether it would like to comment on or be quoted in this story before it is released to the local papers. I have used this approach on two occasions when I have been in the right and in both cases got a quick resolution. They are almost more afraid of bad publicity, particularly if they think it is the kind of story the papers or TV stations might pick up on as a side note on interest, than they are of litigation, which can stretch out for years and nobody ever hears of.
