Richard,

Many times the MM's trade inventory between themselves to fill retail orders. Other times they just up and fill limit orders that are between for who knows what reason.

There are also several automated trading systems, such as Instinet, that allow brokers, dealers, etc. to trade after the open and before the close. Pricing can be a bit more volatile over these avenues at times.
