Starving doesn't involve pain, at least not physical pain.

Huh? Have you ever been on a prolonged fast? I have during some of the anti-war protests, and I can tell you, yes, there IS pain. Not the kind of pain from breaking a bone, but a persistent pain as the body uses its pain mechanism to tell you that something needs attention NOW.
