It would be good if someone would delineate the various reasons for maternal death in the United States. My WAG for the increase from 1991 would be the increase in fertility boosting drugs and in vitro fertilization, performed primarily in older, first-time mothers, creating too many embryoes for the mother to carry safely and thus requiring surgical intervention. But again, that would be just a WAG.

I remember one older woman (over 40 and her first pregnancy) who had gone the in vitro route and was pregnant with quadruplets. She lost one at 6 months, which could be considered a late-term abortion (the fetus had to be extracted to save the others), except that the fetus had tried to deliver itself way too early and had died in utero. The poor woman spent three solid months, flat on her back, in the hospital, to avoid losing her other infants. I am happy to report she was successful in this endeavor. She was also fortunate enough to find and wealthy enough to afford a Rolls Royce brand of medicine.
