I wonder if the date DNA/XOMA decided to start selling Raptiva was coordinated with the date of this NEJM article??

And started selling is a misnomer....aren't a bunch of folks already taking this drug, the ones who participated in the trials and requested to continue on Raptiva?
Who pays for the drug after a trial is over, DNA/XOMA, the participant or their insurance??
