Yes, that's clearly a big issue. Only United Health of the big insurers covers Flumist to my knowledge. Note that Wyeth/MEDI are currently offering a $25 rebate.

In Massachusetts, regular pharmacists currently aren't allowed to administer Flumist because they are not "health care providers." In my search to get it, I called a nearby pharmacy that stocks it (I looked on www.flumist.com ) and the pharmacist was grumbling big time. He said he has it in stock but can't administer it by himself. (He mentioned he actually had a doctor come around with the doctor's own kids to administer it to them). Another obstacle is that it requires storage in a freezer.
