So I would guess that re-activated SARS is the most likely explanation. Wonder if the patient is diabetic or otherwise immune-compromised in some way?

I suppose an alternative might be re-infection via a household pet. Seems very unlikely that live virus hung around all these months in a corner of his house someplace.
