I happen to share the same fear. Tried to cure myself of it by taking an accelerated free fall sky diving class. The instructor told me I should never try it again, lol.

However I do have to fly at least once a month. I have been taking 10 mg of Valium upon arriving at the airport, and it has helped tremendously. It is a dangerous medication, as you can build a tolerance (with accompanying dependency) very quickly, but if you can refrain from using it OTHER than when you are flying, I heartedly recommend it.

Don't even ask me about the Sunshine Skyway bridge, my knees shake every time I go over that sucker.
