Russia did what any sane government would do -- they asked to see evidence of WMD before they would back an invasion. The US said "we know there are WMD for certain", "we know it for a fact"

And apparently the intelligence community believed that did, at least the consensus and/or majority of analysts.

But once again, neither the Russian or US intelligence agencies picked up the fact that Saddam had recommenced his WMD programs surreptitiously under his direct command (thus, eliminating any of the previous chains of command in Iraq).

And who could have known his scientists were so corrupt that they were grifting money off him??

There has NEVER been a war fought in which there was 100% intelligence accuracy, especially when the opposing side is deliberately trying to deceive you..

The Russians did not report that deception, nor did they explain Primakov's presence in Baghdad during the time the war began..
