Ted Kennedy's DIRECT ACTION caused Mary Jo Kopechne's death. Running the car off the bridge was an accident, but he could have tried to save her, he could have run for help and brought them back right away, but no. He thought of no one but himself and ran and hid, and thus showed his character or lack thereof.

You can make an argument that Bush's actions (or lack thereof) indirectly led to 9/11; I personally think that Clinton's actions as Prez were a lot more culpable. I support every one of Bush's DIRECT ACTIONS of the war on terrorism since 9/11.
