Yeah, the cap excludes renewal - yes both extensions (they only ever give 3 years at first) and visas for new employers for existing H1-Bs (I understand you do need to apply for a new visa if you change employer - as the H1-B is specific to employer - mine has my employer written on the thing in my passport). But anyway I think the visas for academics are outside this cap anyway.... I know someone who has fallen into this OPT/H1-B trap....now I understand why he is having a problem.
