I think you're right about the personal relationship and tribal aspects. However, when the next layer down finds their kingpin is disenfranchised, they find a new kingpin. There is no shortage of contenders. When the Gambino family got "restructured" by the Feds in the early 90s, the street level went to Genovese contacts. Later, when the major Italian families all came under pressure, street level shifted to Asian equivalents. Medusa/Hydra, whatever — getting rid of one head just grows another head. Street level will always find a place to source; cutting off heads simply makes room for more heads. There is absolutely no supply-side solution (and not likely a demand-side solution, either, though it's the more promising between the two). Most one can do is legalise, tax and regulate "the pharmacy".
