I agree that in the ideal world, these things would not have happened, but if they did, everybody would have known and tried to stop it.

Instead, there was a cover-up by the Pentagon. Bush did not know, Rumsefeld may or may not have known, he says no, General Myers may or may not have known, he says no.

I certainly didn't know.
