Fiber-to-home: US$40 flat/month with free installation

I had a question from one of the people on SI about fibre to the home in Japan. I thought that might be of general interest, so here is my answer for all of you:

Yes NTT does offer FTTH and I am one of their happy customers. I got it a few weeks ago:

speed: about 100Mbbps (theoretically)
monthly rates: there are different programs, but it's generally about US$ 40/month
Installation at this time is usually free (!)
popular? yes, of course.

If in the US, a carrier would (do they?) offer FTTH 100Mbps at flat rate of US$ 40/month with free installation (including free terminal adapter box and free connection software), do you think this would be popular ?
