Maybe it wasn't just a handful of folks acting on their own. I wouldn't be at all surprised if it went up the command chain a bit. I just don't yet see a reason to conclude that it was a matter of policy.

and regardless of the orders, that responsibilty goes up the chain of command.

To an extent. You don't impeach the president because some private fresh out of basic messes up.

The immediate supervisor and perhaps the local CO would at least be responsible for performing their duties poorly, even if the mistreatment was only by the handful of people that have been accused and had no sanction from the chain of command or from official policies.
