As it so happens I was a F.O. Radio operator (F.O. means forward observer), so I was out with the troops constantly. Personally, I never saw such a thing happen, though I am sure its possible to have accidental shootings into friendlies, BUT every precaution is taken to not have that happen. So far as the artillery is concerned, the radio man calls the coordinates back into FDC (fire directional control) there they plot the coordinates on a huge map that lists all the troop or other friendly placements, then, if all is OK the guns fire (there is an open mike between FDC and the guns so they know exactly what they need to do at the same time FDC is working it up and the FDC radioman is calling the fire mission BACK to the F.O. jdn
