Did you disable System Restore before cleaning the virus/worm. If not, you are probably reinfected. Anti-virus programs can't generally access protected files such as those in System Restore. If that's not it, have you installed, updated and run anything like SpyBot Search and Destroy? You may have been bitten by a browser hijack.
