Yes there was. There was a declaration from the Doctor that she had counseled the patient on the specific risks of the antibiotic in relation to her becoming pregnant. There was also the statement from the Pharmacist that the patient had been given documentation in this regard.
Neither of which was documented, so it comes down to "Your word against mine" with a chunk of cash as the incentive for lying.

And, in relation to this, a take from one of Holly's posts:
Physicians should not rely upon pharmacists or other ancillary personnel to apprise patients of drug hazards and interactions.

Within the last week, I found out - I was told by the doctor during an appt after he he asked when I took a particular drug - that I was taking it at precisely the wrong time. The pill bottle label has printed on it "Take one tablet once a day". No indication of WHEN. I cannot remember EVER being told when to take it by that doctor who prescribed. Nor do I remember receiving written or oral instructions, other than that cryptic note on the pill bottle, from the pharmacist.

The drug aids in preventing a particular malfunction. Now the majority of the time the occurrence of that malfunction simply results in pain. BUT it can have more serious effects part of the time. Including death.

So maybe this counseling may be more important than you seem to think.
