I disagree completely. An utter - indeed, shameless - lack of accountability is a hallmark of government, in all its forms and at every level.

Normal operating actions would dictate that punishments would be handed down for people who didn't do their jobs properly.

You would be correct if you were describing the private sector, where a profit motive, the desire to maintain a certain corporate image or a good reputation, and other factors make...in most cases; yes, naturally there are exceptions...individuals tend to be held accountable for their missteps, intentional or accidental. Government is about statism and political survival, and the sheer size of the bureaucratic leviathan ensures that finger pointing can go on indefinitely with culpability residing everywhere and nowhere simultaneously.

Do you, as I, find it interesting that even as one of the conclusions of the 9/11 committee was that the U.S. intelligence infrastructure is expansive, hindering communication and hampering the making of critical decisions...they advise that we add yet another bureaucratic fixture to the mess?
