Boat crews change. There were perhaps 5 or 6 at any given time, but more than 5 or 6 in total served on Kerry's boat. A wounded enemy with a rocket launcher is more dangerous than an uninjured one. The injured one doesn't care... he knows he's probably going to die anyway... so he's going to do everything possible to take you with him. He's not going to pop, quick fire and run. Again... Kerry didn't award himself the medal... his superiors examined the evidence and signed off on it.
